Just a quick question, flu vaccines – are they also included in the basic price total on the PCSE statement?

The answer is – yes, they are – flu is included in the basic price and you will get a dispensing fee for each flu given.

Also, you get a separate payment through CQRS for administering flu (around £10/patient) but this is paid separately to the vaccine and dispensing fee.

I hope you are keeping well? Just a quick query, our wholesaler rep has advised me last week that we should be purchasing Nebido (£87.11) as a generic (£44.89) due to the considerable price difference, and the drug tariff is paying £108.35 however all of our prescriptions are actually prescribed as Nebido so unable to supply the generic. However, would it be better if I was to ask our doctors to now prescribe this generically, or would you still advise prescribing and ordering Nebido?

I would definitely agree with your rep, the generic is now significantly cheaper than the brand and the generic listing in the tariff has increased massively, so the margin is huge.

So yes, prescribe generically and dispense the generic – until something changes again in the tariff lol.

I’m getting asked by my Practice Manager about the profits when I amend a ‘Generic over Tariff’

Why does Epact refer to ‘Potential spend and Potential saving’?

Is the practice potential saving not the actual total of profit we have made?

The reason Epact says ‘Potential saving’ is because that is the figure that you would have saved the NHS if you hadn’t amended.

Potential spend is what you would have cost the NHS if you hadn’t amended.

Obviously, the NHS would prefer that you didn’t amend but amendments are only done when you are purchasing a generic higher than what Drug Tariff is paying out.
